New Patients

What to bring to your first visit

Please be sure to bring the following items with you to your first appointment:
  • Physicians order or prescription
  • Insurance card - we will need to take a photocopy of the card to insure that we collect all the necessary information to submit to your insurance company.
  • If this is a Worker’s compensation claim, please bring all necessary information you have related to your claim ie. Claim number, adjusters name and contact information.
  • Please bring any completed Coastal Hand and Occupational Therapy paperwork and forms you may have completed if given prior to appointment or downloaded from internet.
  • Any co-payments and deductibles will be collected at the time of service.  We will verify your benefits, including copayment amounts and deductibles as a courtesy at the start of treatment. However, we recommend you also check with your insurance company.
We accept debit cards, cash, check, Visa, MasterCard, and other cards.
What to expect at your first visit....
During your first appointment a Certified Hand Therapist, Occupational Therapist, or Physical Therapist will evaluate your individual circumstances and needs.  Your therapist will take specific measurements such as flexibility, strength, and range of motion and assess any other limitations you may have.  This helps your therapist to better understand your particular needs and outline a treatment plan specifically designed for your situation.  Your treatment plan is designed to help you recover your independence at home and in your work place.
